All Organisations willing to collect funds in Polokwane should please contact the PMTWF office to get a application form which needs to be completed and submitted back to the PMTWF. The following documents will be needed to process your application:
- Completed Application Form
- Completed Admin Expenses Form
- Copy of Collector’s ID (South African Collector) or Passport and Visa (Foreign Collector)
- Organisations Constituition
- Last Two (2) Years Financial Statement
- Details of Your Project (Plans, etc)
- A Letter of Recommendation from a Muslim Organisation in South Africa. The Letter has to be original and approved.
- A Letter of Recommendation from a Muslim Organisation / Aalim in the Area of Origin (Foreign Organisations Only)
